3 = major harm/death
R= Risk rating Likelihood multiplied by severity.
Priority Risk rating decides priority for
action, e.g. 1 = no action 2 = low priority 3 or 4 = medium
6 = high 9 = urgent
Action
required/notes Further precautionary measures that had to be taken
on the day. Specific actions relating to the activity.
Group briefed It is the group
Leader’s responsibility to brief the group about potential hazards and
consequent precautions, initial the boxes when each hazard has been
explained. You may want someone in the group to countersign that all
hazards have been identified. Overall, members are responsible for their
own safety, but Leaders should assess risks to avoid conflict and
prepare better for the visit/activity. A completed risk assessment is
proof that the Leader took reasonable steps to ensure the safety of he
group.
NB
Risk Assessments should be made available to members of the group on
request. It is always advisable to keep a fully stocked First Aid kit.
In some cases, the presence of a fully qualified First Aider is best. Be
aware of any medical conditions/first aid requirements within the group.
